In Mississauga
Halton Region we envision individuals with chronic conditions working as
partners in their own healthcare. We provide programs and services that enable
people to take control of their own health by gaining the confidence, skills
and knowledge necessary to manage the physical, social and emotional aspects of
life with chronic conditions.
To do this, both
individuals and healthcare providers should understand and practice the
principles of Self Management. Self Management workshops can be fully integrated into any clinical pathway including, diabetes, COPD and arthritis.
The Self
Management Strategy is funded through the Ministry of Health and Long Term Care of Ontario and we also have
an Advisory Network of stakeholders (including consumers) from across the
region contributing to our work.
